The crash on the Ohio Turnpike was caused by confusion over the new toll plaza design.

TOLEDO, Ohio — In a heartbreaking interview from the hospital room, Thupten Choedon recounted the crash that killed her husband and 9-year-old son the day after it happened.

"You know that road accident happened because of those?" Choedon said to a sympathetic state trooper.

She was pleading for the redesigned toll plaza on the Ohio Turnpike to be changed.

And now the National Transportation Safety Board (NTSB) agrees.
