Attorney General Keith Ellison announced on Monday that his Medicaid Fraud Control Unit (MFCU) has charged two separate cases alleging over $1.5 million in fraud against the Medicaid program.
Salman Ahmed Elmi is charged with eight felony theft offenses related to Reva Health’s billing for over $1 million in Medicaid-funded services that were not provided or were ineligible for reimbursement.
From 2024 through 2026, Elmi operated Reva Health in Golden Valley, which claimed to provide adult rehabilitative mental health services to Medicaid recipients, the AG’s office said.
According to court records, Elmi and others at Reva falsified documentation, told employees to pay recipients illegal kickbacks in exchange for personal information to support false claims, used ineligible providers to provide services to recipients and falsely claimed to have provided supervision.
